ELI LILLY provides associates with (1) pair of prescription safety glasses and (1) pair of computer glasses annually.  An associate is eligible one year from the date of their most recent order.  A current prescription is required to participate in the program (within 2 years).  If an individual’s prescription exceeds the 2 year requirement, Please visit your local optometrist for a new prescription. Please note: Associates must enter their individual Eli Lilly cost center number to complete the ordering process.  Without the cost center information, the order process cannot be completed.

When does my benefit renew?
Every 12 months, unless you have a change in your prescription or your glasses are damaged in the performance of your work duties.  Please see your supervisor if you need a replacement order in less than the 12-month time requirement

How do I order glasses?
Begin by selecting a frame at the lower portion of this page.   After selecting a frame, click the ADD LENS option to select a lens and any option.   If you don't have your Pupillary Distance (PD) measurement on your prescription, you will be required to upload a measurement photo of your face.   Your prescription can be added to your account by taking a photo of the prescription with your mobile phone.

In the checkout process, please enter Eli Lilly Cost Center and the COMPANY CODE:  Lilly2022.  If you do not know your cost center, please contact your supervisor/manager or visit Ariba for further instructions.

Glasses will arrive in about 10 – 14 days after your order is placed.

Can I order glasses from my home computer?
Yes, you can use a digital camera, webcam or a mobile camera to capture your photo or your prescription.  You must place a calibration strip on your forehead as part of the photo capture process if a Pupillary Distance (PD) is not provided on your prescription.  This will allow our opticians to digitally measure your pupillary distance.  You can print calibration strips from the TAKE PHOTO screens on your local printer.

What if I have a question?
If you have questions about eyewear, benefits, how to use the site or any other question about this program you may contact customer service at 888-314-7786 or email us.  On behalf of the entire SlateBelt Safety Team, we sincerely appreciate the opportunity to serve your colleagues and you.  Have a super day and better tomorrow.
